Home Health

Our department offers home care to the residents of Winnebago County. Our goal is to keep you in your home, safely, for as long as possible.

Skilled Nursing

  • All skilled nursing provided under a physician's order
  • Explain doctors orders
  • Change dressings
  • Injections 
  • Catheter, colostomy care
  • Draw blood for laboratory tests
  • Set-up of medications
  • Assess and evaluate progress following hospitalization
  • Teach families about the disease process of family member
  • Support the family
  • Supervise home health aides
  • County referral and resource
  • Assessments of individuals 60+
  • Home infusion services

Service Limitations:  Agency does not provide ventilation/respirator services, limited daily service of short duration, does not provide after hours or weekend maintenance SN/HCA service, Pediatric infusion under age 18.  Referrals may be declined due to insurance contracts and staffing limitations. Agency will determine if needs can be met by the agency ensuring patient safety and home enviornment safe for agency staff. 

We have nurses who provide skilled services in your home. We offer short-term and long-term care.

Whether you need assistance after coming home from the hospital or you need assistance with your medication setup, we are here for you. Some of the skills we provide are assessments, INRs, blood draws, wound care (including wound VACs), injections, catheter care, colostomy care, teaching, medication management, and others.

Pay is based on individuals' income.

Home Care Aides

We have certified nurse aides that have completed 75 hours of a certified course and have more than 16 hours of training to care for you in your home. Our HCAs provide personal care such as baths/showers, shampoo, shaving, and more.

Medicare, Medicaid, Veterans Administration, State and Federal Grants, and Elderbridge Agency on Aging are some of the resources to help fund the cost of home care. Please call Winnebago County Public Health at 641-585-4763 if you have any questions.

Homemakers

Our homemakers provide light housekeeping which includes vacuuming and dusting, cleaning floors, cleaning bathrooms, laundry, running errands, or any other light cleaning you are not able to do for yourself. Winnebago County Public Health is also certified to participate in a waiver program that provides in-home services-inquire within.

Respite

Our home care aides also provide short-term and intermittent respite for an individual that is providing daily care to a friend or family member.
