Armed Forces Hunting and Fishing licenses are now available to residents of Iowa who served in the armed forces of the United States on federal active duty. To qualify, veterans will need to submit a one-time application for the Iowa Hunting/Fishing License for Resident Armed Forces Veteran along with a copy of their DD214. Stop in today to pick up an application, record your DD214 and learn more about the licenses.

There is no charge to record or obtain a certified copy of a military record. To record your discharge paperwork, you'll need the original discharge paperwork and a photo ID. To request a certified copy, the application below must be filled out completely. If being mailed, the application must be notarized and you will need to include a copy of a driver's license. You will need to show entitlement if the DD214 is not your record.
Veterans and next-of-kin of deceased veterans have full access to the record. Next-of-kin includes the un-remarried widow or widower, son or daughter, father or mother, brother or sister of the deceased veteran.
If you are entitled, please complete the Application for Certified Copy or Photocopy of Military Record form and bring it to our office. We will be happy to complete your record request if entitled to the DD214.
