Passport Acceptance Hours
8:00 - 11:00 a.m. & 12:00 - 4:00 p.m
Our office has one Acceptance Agent; appointments must be scheduled by booking online here or calling 641-585-2094.
First Time Applicants
Items Required for All Applicants
- Passport application DS-11 completed in BLACK INK ONLY
- DS-11
- Print single-sided only; double-sided copies are not accepted.
- Birth certificate (or original citizenship certificate)
- Must be an original or certified copy issued by the state; hospital copies with footprints are not accepted.
- Previously issued passports can also be used in place of a birth certificate; bring both documents to the appointment.
- This document will be mailed in with the passport application and will be returned to applicant up to 8 weeks after new passport arrives.
- Driver's license
- Must be valid.
- If presenting an out-of-state ID, you will need to provide a secondary form of identification.
- Mobile IDs are not accepted.
- Payment
- DO NOT PAY ANY FEES ONLINE!
- Passport fees paid to the U.S. Department of State MUST be paid with a check or money order; NO EXCEPTIONS.
- Photo fee and required execution fee can be paid with check, cash, credit/debit card (+3% for card transactions).
- One passport photo
- Photos available on-site for $15.
Minor Applicants Under 16 Years of Age
- All documents from above are required for minor applicants.
- Reminder: If bringing a previously issued passport as proof of citizenship for the minor, the birth certificate is still required for the appointment to confirm parental relationship.
- Minor applicants must appear in person with both parents.
- Both parents must bring a valid driver’s license.
- Minor's social security number is required on the application. Agent won't need to see the card but parent will need to know the number.
- Ask for details if one parent is unable to appear in person/attend the appointment.
Passport Renewals
NEW! ONLINE RENEWALS
Some applicants may be eligible to renew their passports online. Please visit the travel.state.gov site for information and online renewal instructions. If you are not eligible to renew online, please see information below regarding the mail-in renewal process.
Applicants can renew their passport via mail if they can answer yes to all five statements below:
- You have your passport in your possession to submit with your application.
- Your passport is undamaged other than normal wear and tear.
- Your passport was issued when you were age 16 or older.
- Your passport was issued within the last 15 years.
- Your passport was issued in your current name. If not, do you have proper certified documentation to reflect name change?
How to Renew Via Mail
- Note: The applicant is responsible for mailing the renewal application and materials, not our office.
- Complete renewal application.
- DS-82
- Print single-sided only; double-sided copies are not accepted.
- Submit your most recent passport.
- Include name change documents (if applicable).
- Provide a photo taken within the last six months; our office takes photos for $15.
- Include check or money order. Calculate fees.
- Mail completed application. Mailing instructions can be found on the DS-82 application.
Appointments
- Passport Acceptance Agent is available M-F 8:00-11:00 a.m. and 12:00-4:00 p.m.
- Applications accepted after 3:30 p.m. will be mailed out the following business day.

Fees
- A check or money order is required; it will be sent off as payment to the U.S. Department of State.
- If using a money order, call our office and confirm totals before purchasing money order.
- Payment to the Winnebago County Recorder for the photo and/or execution fee can be paid with check/cash/card.
- DO NOT PAY ANY FEES ONLINE!
- Courier companies, also known as "passport expeditors," may charge unneccesary fees.
- Visit travel.state.gov for important information regarding courier companies.

Passport Photos
Passport photos are available on-site for $15.00.

Additional Information
- All forms must be filled out in black ink only.
- If printing your applications, print them single-sided. Double-sided copies will not be accepted.
- You can track the processing status of your passport online here.
- Supporting documents (birth certificates, old passports, etc.) will be returned to you after your new passport. Documents may take up to 8 weeks to arrive after the new passport arrives.
- Passport cards are NOT VALID for international air travel, visit Get a Passport Card (state.gov) for more information.
